- Lifespan Clinical Medicine: Provide expert primary care, preventive services, and acute treatment for a diverse patient panel across all age brackets, including newborns, pediatrics, adults, and seniors.
- High-Volume Case Management: Manage an active daily schedule of approximately 30–40 patients, masterfully balancing scheduled annual physicals, chronic disease management, follow-ups, and urgent sick visits.
- Autonomy & Prescriptive Authority: Conduct comprehensive physical assessments, formulate precise diagnoses, order and interpret diagnostic/lab testing, and prescribe medications in strict accordance with NYS regulations.
- Chronic & Preventive Care: Lead interventions and treatment plans for high blood pressure, diabetes, high cholesterol, asthma, and COPD while championing health maintenance and routine screenings.
- Patient & Family Education: Educate parents, children, and adult patients on treatment adherence, disease prevention, and individualized wellness goals to foster comfortable, positive healthcare experiences.
- Cross-Site Collaboration: Partner closely with our Family Medicine Physicians, Medical Assistants, and Office Managers, while maintaining flexibility to provide coverage across our Brooklyn clinic locations as needed.
- EMR Integrity: Maintain accurate, thorough, and timely medical documentation for all patient encounters.
Schedule & Location
- Hours: 5 days a week, day shift (9:00 AM – 5:30 PM).
- Weekend Rotation: Schedule includes one weekend day per week, with a dedicated compensatory day off provided during the week.
- Location: Brooklyn, NY 11220 Clinic, with regional travel to provide coverage at additional Brooklyn sister sites when required.
- Transit/Parking: Highly accessible via public transit (Please note: street parking only; no dedicated facility parking available).
Qualifications Education, Licensure & Certifications
- Active New York State Nurse Practitioner (NP) License and current registration.
- Family Nurse Practitioner (FNP) Certification via ANCC or AANPCB.
- National Board Certification (BC).
- Active DEA Registration with full prescriptive authority for controlled substances.
Experience & Skills
- Minimum 3+ years of clinical experience as an FNP managing patients across the entire lifespan (ages 0 to 100 required).
- Language Proficiency: Must be fully Bilingual in Spanish and English to effectively communicate with our patient population.
- Exceptional interpersonal skills with a proven track record of making pediatric and adult patients feel at ease.
Commitment
- Candidates must be prepared to sign a 2-year commitment contract which features a standard $30,000 restitution clause for early termination.
