Ideal Candidate:

• Licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) in the state of Texas
• Minimum 1 year of experience preferred (home health experience a plus)
• Strong communication and documentation skills
• Reliable transportation and willingness to travel within your chosen territory
• Passion for patient care and autonomy in a home health setting

Job Duties:

• Provide skilled physical therapy or assistant-level services to patients in their homes
• Develop and implement individualized treatment plans (PTs)
• Work closely with patients, caregivers, and other clinical staff
• Complete accurate and timely documentation
• Promote patient independence and recovery through evidence-based care
