In this role, you'll provide exceptional speech-language pathologist (slp) services in the comfort of patients' homes, focusing on functional improvement, independence, and quality of life enhancement. Key Responsibilities
  • Communication Assessment: Conduct comprehensive evaluations of patients' speech, language, voice, fluency, and cognitive-communication abilities in their home environment
  • Swallowing Evaluation: Perform clinical swallowing assessments and develop safe eating and drinking recommendations to prevent aspiration
  • Treatment Planning: Develop individualized treatment plans with measurable goals focused on improving communication abilities, swallowing safety, and cognitive function
  • Therapeutic Interventions: Implement evidence-based speech therapy techniques including articulation therapy, language intervention, voice therapy, dysphagia management, and cognitive rehabilitation
  • Patient & Caregiver Education: Provide expert guidance on communication strategies, safe swallowing techniques, diet modifications, and home practice activities
  • Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments
  • Care Coordination: Collaborate effectively with physicians, nurses, dietitians, and healthcare team members to ensure comprehensive patient care
  • Progress Monitoring: Regularly reassess patient progress, adjust treatment plans as needed, and ensure adherence to clinical standards and regulatory requirements

Required Qualifications Licensure & Certifications
  • Graduate of an accredited Speech-Language Pathology program (Master's degree required)
  • Valid Illinois Speech-Language Pathologist license
  • ASHA Certificate of Clinical Competence (CCC-SLP) or Clinical Fellowship eligibility
  • Current CPR certification
  • Valid driver's license and auto insurance

Experience & Skills
  • Open to new graduates and experienced speech-language pathologists - comprehensive training provided
  • Willingness to learn WellSky Kinnser EMR system (training included)
  • Strong clinical assessment and critical thinking abilities
  • Outstanding written and verbal communication skills
