Community Care Practitioner

We are recruiting compassionate, reliable and professional Community Care Practitioners (FTMs) to join our team.As a CCP, you will support clients to live independently, safely and with dignity while delivering high quality, person-centred care.Key Responsibilities:Deliver care in line with individual Care & Support Plans Support clients with personal care, medication and daily livingAct as a Key Worker, building meaningful relationships and supporting positive outcomesMonitor and identify changes in clients’ needs, risks or wellbeingReport and escalate any concerns immediately to your Senior Care CoordinatorMaintain accurate, clear and timely care recordsRequirements:Caring, professional and reliable approachGood communication skillsAbility to recognise and escalate concerns promptly Full UK driving licence and access to a vehicle (essential due to rural locations)Right to Work in the UK - Minimum of 3 months remaining on RTW.Sponsorship (UKVI Compliant):The Divine Group may offer Skilled Worker visa sponsorship to candidates already residing in the UK. Applicants must have a minimum of 4 months remaining on their current visa and must meet all Home Office eligibility requirements. Sponsorship is subject to role availability and compliance with UKVI regulations.The Divine Group does not charge any fees for sponsorship. Applicants must not make any payments to third parties or agents for a Certificate of Sponsorship (CoS).
