- Conduct assessment, analysis and treatment plan.
- Perform nursing services for chronic illness, disability or home bound patients.
- Recommend, instruct and teach patients.
- Counsel patient and family to meet treatment plan goals.
- Establish Plan of care for each and every patient and follow plan of care.
Requirements:
- State of Michigan RN License
- Valid Drivers License
- Auto Insurance
- Home Care experience Preferred
