Tend Health is a New Zealand "digi-physical" healthcare provider that combines in-person medical clinics with 24/7 digital healthcare services through its app and online platform.Health Improvement Practitioner (HIP) is a registered health professional who works within general practice clinics to help people improve their mental health, physical wellbeing, and lifestyle through brief, evidence-based interventions. HIPs are a key part of the Te Tumu Waiora primary mental health model. Job Description Typical responsibilities include:Providing brief interventions for mental health, stress, anxiety, addiction, and wellbeing concerns.Working with patients of all ages, individuals, and whānau (families).Supporting behaviour change related to health and wellbeing goals.Collaborating with GPs, nurses, health coaches, and other primary care staff.Assisting patients to develop practical action plans.Delivering evidence-based approaches such as behavioural health consultation and brief psychological interventions.Helping general practice teams improve their capability to support mental health and addiction concerns.Promoting equitable health outcomes, particularly for Māori and other priority populations.Pay: $7,100.00 – $7,900.00 per monthBenefits:
