- Assess clients and develop person centred care plans and risk assessments in collaboration with healthcare professionals and families.
- Ensuring delivery of personalised care to HOME CARE CLIENTS in accordance with their individual care plans.
- Supervise and support junior staff members, fostering a positive team environment
- Monitor accuracy of records of care provided using IT systems and ensure compliance with regulatory standards.
- Promote a safe and comfortable environment by adhering to health and safety policies.
- Support clients' emotional well-being through empathetic communication and companionship.
- Contribute to organising activities that enhance residents' quality of care.
- Using spot checks to identify training needs and key policies and procedures to support staff improve practice.
- Carrying out client medication stock checks, ensuring you check expired, discontinued medication, correctly log on system, and return them to pharmacy.
- Communicate effectively with pharmacies, GPs health care professionals, staff and clients regarding medication changes and concerns.
Skills
- Experience in supervising care and an ability to lead and motivate a team effectively.
- At least NVQ 3 or willingness to complete the training up to NVQ Level 5 in Health and Social Care.
- Strong leadership skills combined with excellent communication abilities
- Knowledge of creating, implementing, and reviewing care plans tailored to individual needs
- Proficiency in using IT systems for documentation and reporting purposes
- Valid driving licence is advantageous for supporting transportation requirements
- Good organisational skills with the ability to prioritise tasks efficiently
- Compassionate nature with excellent interpersonal skills suitable for working with elderly residents
