Community Carer

OverviewWe are seeking a compassionate and dedicated Community Carer to join our team. The ideal candidate will provide essential support to individuals within their homes or care settings, promoting independence and well-being. • Assist clients with daily activities such as personal care, meal preparation, and medication reminders
• Support clients in maintaining their independence within their homes or care environments
• Drive clients to appointments, social activities, or errands using company vehicles or personal transport where appropriate
• Develop and follow personalised care plans tailored to each individual’s needs
• Monitor and report on clients’ health status and wellbeing, ensuring any concerns are communicated effectively to relevant professionals
• Maintain accurate records of care provided and any changes in client conditions
• Promote social interaction and emotional support to enhance clients’ quality of life
• Adhere strictly to health and safety guidelines, including infection control procedures

Skills

• Experience working in a care home or providing home care is desirable but not mandatory; training will be provided where necessary
• Ability to develop and implement personalised care plans effectively
• Strong communication skills in English, both verbal and written, to interact confidently with clients and team members
• Proficiency in using IT systems for record keeping and reporting purposes
• Excellent interpersonal skills with the ability to show empathy, patience, and understanding towards clients’ needs
• Good organisational skills to manage schedules efficiently and prioritise tasks effectively
