Your Impact & Responsibilities:
- Comprehensive Evaluations: Perform high-quality physical therapy evaluations, developing personalized treatment plans for post-acute and long-term care residents.
- Skilled Interventions: Implement evidence-based treatments to improve resident mobility, balance, strength, and overall functional independence.
- IDT Collaboration: Partner closely with nursing, therapy, and medical providers to ensure a holistic approach to resident recovery.
- Expert Documentation: Maintain precise and timely documentation in accordance with California state and federal regulatory requirements.
- Patient Advocacy: Educate residents and their families on individualized therapy programs, safety protocols, and long-term functional goals.
- Clinical Leadership: Foster a positive, team-oriented therapy environment focused on achieving the highest possible outcomes for our residents.

Qualifications:
- Licensure: Current, valid California Physical Therapist (PT) license in good standing.
- Skills: Strong communication, clinical reasoning, and a passion for serving the geriatric and post-acute populations.
- Experience: Previous SNF experience is a plus, but New Graduates are encouraged to apply!
