Responsibilities
This role is complementary to, and not a substitute for, GPs. GPs remain responsible for overall clinical leadership and holistic management of complex patients.About the PracticeTal-y-bont surgery is a friendly four partner training GP practice based on the boundary between Swansea and Carmarthenshire. Although we are a Swansea university health board practice, one third of our patients are residents of Carmarthenshire. We have a list size of approximately 9800 patients. We are a close-knit team of three GP partners, one pharmacist partner, two salaried GPs, a mental health practitioner, a women's health specialist nurse, two practice nurses and a HCA. We are also an established training practice currently training two GP registrars. We also have the benefit of an in‑house pharmacy team of two pharmacists along with pharmacy trainees and are supported by an experienced friendly team of admin staff, receptionists, head of operations and a practice manager. - Assess, diagnose and manage patients presenting with acute problems on the day, within an agreed scope of practice and competence.
- Provide some planned reviews for stable long term conditions and selected home visits, as agreed in the job plan.
- Work autonomously but with readily available GP supervision, escalating complex or high risk cases appropriately.
- Help manage GP workload safely and effectively, while ensuring that GPs retain responsibility for undifferentiated, complex and high risk presentations.
