Job Overview

To support service delivery, governance, and sustainability, Hywel Dda UHB will appoint a 22.5 hour Lead Pharmacist to provide professional leadership, clinical oversight, and operational management of the Clinical Streaming Hub Pharmacist role(s).

The Lead Pharmacist will provide strategic, clinical, and professional leadership for pharmacy input into the Clinical Streaming Hub, ensuring safe, effective, and consistent medicines optimisation within urgent care streaming.

The post holder will:
  • Manage and support the Band 8a Clinical Streaming Hub Pharmacist(s)
  • Ensure pharmacy contribution aligns with UEC, frailty, and community ‑ based care priorities
  • Provide senior clinical expertise and professional governance
  • Lead development, evaluation, and sustainability of the pharmacy streaming model

Main duties of the job

Provide senior pharmacy leadership within the Clinical Streaming Hubs, acting as the responsible professional lead for Hub pharmacists. Ensure safe, consistent, evidence‑based medicines optimisation and provide expert advice on complex medicines‑related streaming decisions.

Line manage, appraise, and supervise Band 8a pharmacists, supporting professional development, advanced practice progression, and competency assurance. Ensure safe, sustainable clinical cover and foster a high‑performing pharmacy workforce.

Lead development and review of SOPs, pathways, and protocols for pharmacy streaming, ensuring compliance with GPhC standards, NHS Wales policy, and Hywel Dda governance frameworks. Contribute to clinical risk management, incident review, audit, service evaluation, and QI.

Act as senior pharmacy link between the Clinical Streaming Hub and primary care, community pharmacy, secondary care, and frailty/community response services. Lead education and mentorship for pharmacy staff, supporting non‑pharmacy clinicians with medicines and frailty‑related education.

Contribute to development of advanced practice roles and provide senior pharmacy input to service planning, escalation, and transformation. Support operational resilience during high demand or service change.

Accountable to the Director of Pharmacy/Hywel Dda pharmacy leadership, aligned operationally with Clinical Streaming Hub and UEC structures, with line management responsibility for pharmacist(s).

  • Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 99 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
  • Numerous mental health and learning disabilities services
