Job Overview

The Clinical Lead Nurse provides visible, strategic and professional leadership across primary care nursing services within Hywel Dda UHB. The role focuses on ensuring high-quality, safe, and effective patient care, while supporting delivery of organisational service, performance, and quality objectives.

The post holder has responsibility for the professional and operational management of nursing teams within Health Board Managed Practices, including workforce oversight, performance management, and staff development (e.g. revalidation, appraisal, training).

Working autonomously and collaboratively, they:
  • Lead service improvement, audit, and governance, using evidence-based practice
  • Support and develop nursing teams through coaching, mentoring, and clinical guidance
  • Act as a role model for best practice and professional standards (NMC)
  • Assist struggling practices and drive consistent standards across primary care
  • Contribute to the strategic development of primary care services aligned with Health Board and national priorities
  • The role requires strong leadership, communication and change management skills, working across multi-disciplinary teams and systems to improve patient experience, workforce capability, and service delivery outcome.

Main duties of the job

  • Provide visible, accessible leadership to primary care nursing teams across managed practices.
  • Lead on quality, safety, governance, and patient experience improvements
  • Manage and support nursing workforce, including recruitment, appraisal, revalidation, and performance issues.
  • Oversee and contribute to service development, policy implementation, and standard setting.

Monitor and improve clinical practice using audit, data analysis, and evidence-based approaches.
  • Support struggling practices and drive consistent standards across services.
  • Facilitate staff development through coaching, mentoring, and education pathways.
  • Work collaboratively with multi-disciplinary teams across primary, community, and secondary care.
  • Contribute to strategic planning and transformation of primary care services.
  • Lead and manage complaints, incidents (Datix), safeguarding, and investigations.
  • Ensure effective communication of complex and sensitive information with staff, patients, and stakeholders.
  • Support financial and resource management, ensuring efficient use of budgets and workforce.
  • Promote and implement infection prevention, governance standards, and organisational policies.
  • Act as a role model for professional practice, maintaining clinical credibility and NMC standards.
  • Lead and participate in training, service innovation, and change management initiatives.

Working for our organisation

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our services are provided in:
  • Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest
  • Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
  • Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
  • 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 99 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
  • Numerous mental health and learning disabilities services
