Care Manager (Registered Nurse) Visionwest Home Healthcare Bay of PlentyVisionwest Home Healthcare Bay of Plenty is a non-profit healthcare organisation providing in-home aged care, disability support, rehabilitation, respite care, and home assistance services throughout the Bay of Plenty region. We are seeking an experienced and compassionate Care Manager (Registered Nurse) to support the delivery of high-quality community healthcare services. About the RoleAs a Care Manager (RN), you will oversee client care within the community, ensuring individuals receive safe, effective, and person-centred support while maintaining their independence at home. This role combines clinical assessment, care planning, coordination, and leadership within a multidisciplinary healthcare environment. Key Responsibilities

• Conduct comprehensive client assessments and identify care needs
• Develop, implement, and review individualised care plans
• Provide clinical guidance and support to Support Workers and care staff
• Monitor medication management and ensure clinical compliance
• Coordinate services with clients, families/whānau, healthcare providers, and community agencies
• Maintain accurate clinical records and documentation
• Ensure services meet quality, contractual, and regulatory requirements
• Respond to client concerns and changing healthcare needs
• Participate in quality improvement initiatives and clinical audits
• Support staff training, mentoring, and professional development

Requirements

• Current New Zealand Registered Nurse (RN) practising certificate
• Experience in community, aged care, disability, or home healthcare nursing
• Strong assessment, care planning, and case management skills
• Excellent communication and relationship-building abilities
• Full New Zealand driver’s licence and willingness to travel within the service area
• Knowledge of interRAI assessments is advantageous
